Series i Change Out Request

The template for the iSeries Change Out Request Form can be found below. If your company meets the requirements listed at "RJS Software Product Change Out Requests," you may feel free to use this template to request a new permanent access code for your RJS Software Systems iSeries products.

Since RJS Software Systems licenses the its iSeries product libraries based upon Serial Number, Model Number and Partition Number (also known as LPAR), this information will be necessary to complete the form below. If you need assistance on this, please visit "Finding Your iSeries System Information."

Formatting and Submission Requirements

RJS Software Systems will be looking for the following when processing a Change Out Request:

Requests that do not meet the Maintenance/Removal Requirements or the requirements listed above will be rejected.

Formatting Requirements

• The request must be completed in full and the system information, old and new, must be correct.
• The request must be submitted on your company letterhead.
• The request must be signed.

Submission Requirements

Once the request has been filled out, it may be submitted via the contact information below:

  • PDF or other image format is required.
• Fax to (952) 736-5801.
