The template for the PC Change Out Request Form can be found below. If your company meets the requirements listed at RJS Software Product Change Out Requests, you may feel free to use this template to request a new permanent authorization code for your RJS Software Systems PC products.

RJS Software Systems PC product licenses are based upon two values called the Code Entry Number and Computer ID Number. These numbers will be unique on each and every system. To complete the form below, you will need to know the Code Entry Number and the Computer ID Number from both the new and the old PC Systems. If you need assistance with locating this information, please visit "Finding Your PC Licensing Information."

RJS Software Systems will be looking for the following when processing a Change Out Request:

• The request is completed in full and the system information, old and new, is correct.
• The request uses your company letterhead.
• The request is signed.

Once the request has been filled out, it may be submitted via the contact information below:

• Email to support@rjssoftware.com
  • PDF or other image format is required.
• Fax to (952) 736-5801.

Requests that do not meet the Maintenance/Removal Requirements or the requirements listed above will be rejected.
