Problem:
The first problem is that the calendar applet "myLittleTools.net" does not work. Clicking "Choose this Date" does nothing. The second is that the expiration date on the user screen cannot be updated. Included below are the screenshots of the issue.

Solution:
There were several typos in the ASP page mlcpopup.asp that prevented this form and applet from being fully functional. These were corrected in the fixed version of the page below. Please download the self-extracting zip file below to receive the fixed version of mlcpopup.asp.
